By booking a consultation with Donna Sadzius Homeopathy you agree to the terms listed below:
Donna Sadzius Homeopathy provides a platform for you to meet online via Telehealth.
Your agreement of consultation will be directly with Donna Sadzius. Please check you are happy with all of the details before you proceed with a consultation. Once your completed history has been returned, this will be the starting point of a contract between you and Donna Sadzius. Donna Sadzius Homeopathy holds no liability for your ‘no-shows’ and will not be liable to repay any funds received for your ‘no-show’.
Homeopathy alongside mainstream Healthcare:
Homeopathy works well alongside mainstream healthcare programs and medications. You will not be asked to come off your Doctors medications during your treatment. You may be asked to go for additional health checks to see if your medications are still required.
Homeopathy is not instead of Accident and Emergency Care:
If you need emergency health care please contact your Doctor or Emergency Services. Donna Sadzius Homeopathy cannot provide an emergency service. If you need emergency care please contact your nearest emergency services.
Booking:
Your agreement will be directly with Donna Sadzius Homeopathy.
When you have decided to have a consultation with Donna Sadzius Homeopathy, you will fill in the initial history form with the details asked and return to Donna Sadzius. This is the start of your contract of service. You will then book your appointment time via the website, donnasadzius.com.
Payment:
The booking system will take your full payment via credit card using the Square payment processor at checkout. Our payment currency is AUD and will convert to your country of origin upon payment if not from Australia.
The Consultation:
The Consultation will take place at the time indicated if not immediately. You may be asked for photographs to see skin complaints.
Pharmacy:
You will be sent written instructions for your remedies after our consultation. Your remedies are available for purchase via Donna Sadzius’s dispensary and can be picked up from the Palmwoods clinic if living locally, or posted at your request. You will be sent details of where to purchase them if you are an international client.
Confidentiality:
Donna Sadzius Homeopathy will not share your details with anyone. Please ask Donna Sadzius for more information if required.
The consultation will not normally be recorded, but if for some reason it needs to be, it will be held in a secure unit according to the data protection act.
You can ask for a copy of this consultation, but please be aware that we will not be responsible for the security of the email service to deliver it to you.
Emails:
When you book a consultation using this service, your name and details will be added to Donna Sadzius Homeopathy database so that we can send you the remedy instructions, and invoice if required, and out occasional newsletter.
You can opt out to receive details about our services news and courses, and about learning more about Homeopathy. You can also unsubscribe from this at any time.
Health Notes:
Donna Sadzius Homeopathy is required by law to keep your consultation notes for 7 years. These will not be shared with anyone unless the law requires me to submit information for the protection of life.
The Consultation:
Everything discussed at each consultation is confidential and will not be discussed with anyone else unless we have your specific instruction to discuss your information with other health professionals.
Code of Conduct:
I abide by the Code of Conduct set out by ARoH of Australia.
Technology:
With the use of Internet, video calls, messenger, emails or telephone calls every precaution reasonable possible will be taken to ensure the privacy of your information but it cannot be guaranteed across these technologies.
Follow Up Consultations:
This will be discussed with Donna Sadzius Homeopathy to ensure the best way forward.
